In the Champions Cup 1968-1969, Milan participated as the champion of Italy. In the first round they may have lost to Swedish Malmö FF in the first game 2-1 but with 4-1 in the rematch at the San Siro, with two goals by Pratti and one by Sormani and Rivera, the Rossoneri advanced to the next round.

In the round of 16, Milan would face the winner from the pair Ruch Chorzów- Dynamo Kyiv. The Operation Danube, the joint invasion of Czechoslovakia by four Warsaw Pact states, the Soviet Union, Bulgaria, Hungary and Poland, in August 1968 forced the teams of these countries to cease all militant activity. As a result, Milan went without a match in the quarterfinals.

Pratti gave Milan the lead from 7' while in 40' with his second goal it became 2-0. Ajax tried to get back in the game by reducing Vasovic with a penalty to 2-1. However, first Sormani and then Pratti, who scored a hat-trick, made the final 4-1, giving the second Champions Cup in Milan in a triumphant manner.

Pierino Pratti was one of Milan's best players that season. In the Champions Cup he scored 6 times as many as Cruyff, and was the second best striker after Dennis Low of Manchester United who scored 9. In addition, in Serie A he scored 14 more goals and Milan finished in 3th place in the league. This jersey is an exact copy of the one worn in the 1968-1969 Champions Cup. As usual in Italian teams, the emblem was replaced by the Italian logo as Milan was the holder of the Italian championship. In addition, this was one of the few times that Milan used the Rossoneri shirt in the final and not the gourd, as they believe, white.
